Measures
How we measure the effectiveness of our Community Based Programs
Health in Mind provides community-based coaching services for consumers within our local community. Our staff offer Parent Coaching, Independent Living Coaching for youth and adults, as well as Supervised Visitation support.
Our office focuses on overall family wellness. Using pre-and post-program evaluation tools, consumers in our Parent Coaching program are invited to identify their sense that they are:

In charge of their household
-
equipped to manage their children’s needs
-
connected with their child
-
able to provide structure in their home
-
empowered to balance care for self and others, and
-
knowledgeable about parenting resources in their community.
Outcomes are also measured by an overall decrease in their need for community interventions and an increase in self-care and goal attainment.
Specific benchmarks include decreases in the following areas:

family involvement with residential services
-
child involvement with Department of Juvenile Justice or court services
-
child involvement with academic disciplinary concerns
-
family involvement with Child Protective Services, domestic violence services, or substance use
-
child mental health concerns or substance use
Parental connection with appropriate mental health services and overall goal achievement also provide key indicators of success.
For consumers in our Independent Living Coaching program success is measured by maintenance of:

employment or academic enrollment
-
natural resource development
-
housing stability
-
financial management
-
attention to mental health/medical needs

transportation stability
